What to do when there is an accident:

If you are hurt in a traffic crash, do not ride away or shake off what seems like a minor injury — you might find later that it is worse than you thought.

  1. Call 911, so a police officer can make a report. 
    If needed, get medical help immediately.

     
    If injured, do not move until an EMT has had a chance to examine you. Ask the driver or a witness to call 911.
    Stay calm so you can ask questions and take notes.
  2. Acquire this information from the Driver:
     
    Driver's Name Driver's Address
    Driver's Phone Number Driver's License Number
    License Plate Number of Car Make of Car
    Insurance Company Insurance Policy Number
  3. Acquire this information from Witnesses:
     
    Witness's Name Witness's Phone Number
  4. Acquire this information from the Police Officer:
     
    Officer's Name Officer's Badge Number
    Police Report Number
     
  5. Write down how the crash happened while it is fresh in your memory.

  6. Keep (or photograph) any damaged clothes or equipment.
