Please fill out this Report of Favorable Police Conduct Form as completely as you can. It is important for you to provide as much information as possible to help ensure that the favorable conduct of the police officers is fully documented.
Please be sure to fill in the facts of the favorable police conduct including:
- The date and time of the incident.
- The officer's name, badge number, unit of assignment or car number.
- Your address and phone number and any addresses where you can be reached. (This is not necessary but would be appreciated.)
When you have completed the form, click on the Submit button.
Your form will immediately be emailed to the Cincinnati Police Department. The reports are an integral part of the mutual accountability plan set forth in ¶30 of the Collaborative Agreement to "ensure that the conduct of the City, the police administration, members of the Cincinnati Police Department and members of the general public are closely monitored so that the favorable and unfavorable conduct of all is fully documented and thereby available as a tool for improving police-community relations under the CA."
