OFFICE HOURS
You may apply for certificates in person, Monday through Friday, 8:30 a.m. until 4:00 p.m. Certificates may be picked up over-the-counter on the day applied for between the hours of 8:30 a.m. and 2:00 p.m.  Orders placed between 2:00 p.m. and 4:00 p.m. will be mailed. We no longer accept credit card payment for in-person order.  To place an order using a credit card, please call toll- free at: 1-866-550-1853.  An additional $9.95 transaction fee is added to process your credit card order.


Vital Records opens at 10:30 a.m. on the first Tuesday of each month. We are closed on all Federal Holidays.

COSTS 
Birth and Death certificates are $22.00 per copy.
Plastic Birth Registration Cards are no longer available.

HOW TO APPLY FOR BIRTH OR DEATH CERTIFICATES
Certificates are public records and may be obtained by providing us with the necessary search information. You do not need identification to obtain a certificate. All certified copies have a raised, embossed seal of the State of Ohio. You can apply: in person, by mail,  telephone, online or call 1-866-550-1853 for telephone orders using a credit card.  An additional $9.95 transaction fee is added to process your credit card order.

To order In-person:  Elm Street Health Center, 1525 Elm Street, first floor – one block north of Music Hall. Certificates may be picked up over-the-counter on the day applied for between the hours of 8:30 a.m. and 2:00 p.m. Orders placed between 2:00 p.m. and 4:00 p.m. will be mailed. We accept cash, money orders, and certified bank checks. 

To order by mail : We accept money orders and certified bank checks made payable to:Treasurer, City of Cincinnati. Include postage or a self-addressed, stamped envelope in which to return your certificate. 
Birth Certificate Application  and/or  Death Certificate Application

To order Online: Go to  www.Vitalchek.com, click on the State of Ohio, then select the City of Cincinnati. An additional $9.95 transaction fee is added to process your credit card order. UnlessUPSExpress delivery is requested, telephone, and online orders will be returned via regular U.S. mail. Please allow up to twenty working days from the time we receive your request until you receive your order. There is a $3.00 non-refundable search fee on all records not found. All refunds take approximately six weeks to process.

GENEALOGICAL RESEARCH
Genealogists are permitted to come to our office to do research by appointment only. Appointment hours are from 9:00 a.m.to 11:00 a.m. and 2:00 p.m. to 4:00 p.m., Tuesday, Wednesday and Thursday. Please call (513) 352-2912 to schedule an appointment.  The Vital Statistics Department of the Cincinnati Health Department has records starting in 1909. Records prior to 1909 are located at:

LOCATION
We are located at 1525 Elm Street in the Elm Street Health Center building on the first floor.  We are two blocks north of Music Hall in downtown Cincinnati, Ohio. 

DIRECTIONS
From I-75 going south: Take the Ezzard Charles exit and turn left. Follow Ezzard Charles until you get to Central Parkway. Make a left turn onto Central Parkway. Go three blocks - we're on the right hand side of the street in a red brick building that has 'Elm Street Health Center' on it.

From I-75 going North from Kentucky: Take the 5th Street exit (exit is off to the right). Turn left onto Central Avenue, going north. Travel on Central Avenue for five blocks and turn right onto Ezzard Charles Drive, going east. Follow Ezzard Charles Drive until it dead ends at Central Parkway and make a left turn at the light. Go 2 1/2 blocks north on Central Parkway to Odeon Street. The parking lot is located at the corner of Central Parkway and Odeon Street. The Building has Elm Street Health Center on it. 

From I-71: Going south, take the Reading Road exit and turn right onto Liberty Street. Proceed down Liberty Street until you get to Central Parkway. Turn left and stay in the left lane. At the first light, which is 14th Street, make a left U-turn onto the other side of Central Parkway. Go three blocks and turn right into the parking lot directly behind our red brick building.

From I 471 going North from Kentucky: Take I-471 north to the Liberty Street exit. Proceed down Liberty Street until you get to Central Parkway. Turn left onto Central Parkway and stay in the left lane. At the first light, which is 14th Street, make a left U-turn onto the other side of Central Parkway. Go three blocks and turn right into the parking lot directly behind our red brick building.
