The contractor or subcontractor must provide written notification to each current and new employee working on living wage contracts or agreements.  The Living Wage Employee Notification form must be signed by both the employee and the contractor or subcontractor.  A copy of the form must be retained by contractors and subcontractors and be subject to audits and/or other forms of monitoring.
