The parking fee for employees of the City of Cincinnati is $45.00 per month if parking in the Hennegan Lot and $20.00 per month for all other lots. If you wish to alter your parking status, please complete and print this page, including signature. Return the form to Debbie Armstrong, Room 202 City Hall.

Forms without an original signature will be discarded.

This authorizes the payroll deduction of parking fees in the amount determined by the City Manager beginning with the effective month listed above. I understand the advance deduction will be taken in the second pay period of the preceding month.
 
TERMINATION
If termination is requested, the unexpired pass must be returned to the Office of the Treasurer by the second working day of the month or the parking payroll deduction will continue.
