Reporting Complaints, Fraud, Waste, and Abuse Online
Please use the form to report incidents for complaints against Cincinnati police officers as well as fraud, waste, abuse, and ethical misconduct by City of Cincinnati employees.
This form is only meant to be used to report allegations of specific incidents. It is not meant to be used to report issues such as sexual harassment, equal employment opportunity related issues, or other grievances better reported to Human Resources.
Please be aware that any investigations that come out of tips can only be as good as the information provided. Be as complete as possible with any information you have. If you would like feedback on a complaint, but wish to remain anonymous there are free email services available on the Internet that you could use to communicate with the CCIA.
Other Options To Report
- In Person: Come to 805 Central Ave., Suite 610, Monday through Friday from 8 a.m. to 4:30 p.m. The staff can respond to questions and submit a complaint form for you.
- Telephone: Call CCIA at 513-352-1600. To report fraud, waste and abuse of city resources, call the Fraud, Waste and Abuse Hotline at 866-578-2345.
- Email: Submit your CCA complaint to CCAfirstname.lastname@example.org. Be sure to include your name, phone number, and mailing address. Submit your fraud, waste or abuse concern to email@example.com.
